Local Anaesthesia

Topical anaesthetic cream applied to the buccal mucosa (inner cheek lining) followed by a local anaesthetic injection. The procedure is performed entirely under local anaesthesia — no general anaesthesia, no sedation. The area is completely numb within minutes.

Intraoral Incision & Suturing

A tiny incision (2–3 mm) is made inside the mouth on the inner cheek — no external incision, no visible scar. A small circular core of the buccinator muscle and subcutaneous tissue is removed. An absorbable suture (Vicryl) is passed through the dermis, around the muscle, and tied — tethering the skin inward to create the dimple indentation.

Immediate Result & Aftercare

The dimple is visible immediately — even at rest — due to post-procedure swelling. As swelling resolves over 2–4 weeks, the dimple transitions to appearing only when smiling, which is the natural look. Chlorhexidine mouthwash used for 1 week. Soft diet for 3–5 days. The entire procedure takes 20–30 minutes.

Recovery Guide

Week by Week Recovery

Dimpleplasty recovery is minimal. Most patients return to work the same day. The main adjustment is understanding the early at-rest dimple appearance — which is temporary and transitions to a natural smile-only dimple within 2–4 weeks.

Day 1–3

Mild swelling inside the cheek. Dimple visible even at rest — this is normal and temporary due to oedema tethering the skin. Mild soreness managed with paracetamol. Begin chlorhexidine mouthwash.

Day 3–7

Swelling gradually reduces. Dimple still present at rest but beginning to soften. Soft diet continues. No vigorous mouth movement or strenuous exercise.

Week 2

Most swelling resolved. Dimple now primarily visible when smiling — the natural dimple behaviour. Internal sutures beginning to dissolve. Presentable for social settings.

Week 3–4

Dimple fully transitioned to smile-only appearance. Fibrosis (scar tissue) forming between the dermis and underlying muscle — this is what makes the dimple permanent over time.

Month 1–3

Final result established. The fibrotic adhesion between skin and buccinator muscle is mature. Dimple appears naturally with smiling and facial movement. Permanent result.
