Permanent Dimples. 20 Minutes. No Scars.
Dimpleplasty — also called dimple creation surgery — is a minor surgical procedure that creates permanent dimples on the cheeks or chin by forming a controlled adhesion between the skin and the underlying buccinator muscle.
Natural dimples occur when a cleft or bifurcation in the zygomaticus major muscle tethers the overlying dermis — causing the characteristic indentation during smiling. Dimpleplasty replicates this mechanism surgically using a small intraoral incision and an absorbable suture.
The procedure is performed entirely inside the mouth — no external incision, no visible scar — under local anaesthesia in under 30 minutes. Patients resume normal activities the same day.
Which Option Is Right for You?
Cheek dimples, chin dimple or a non-surgical trial — placement and technique are selected at consultation based on your facial anatomy and aesthetic goal.
Bilateral Cheek Dimpleplasty
Most PopularDimples created symmetrically on both cheeks at the same appointment — the most requested configuration. Placement is mapped to the intersection of a vertical line through the lateral canthus (outer eye corner) and a horizontal line through the commissure (corner of the mouth), following golden ratio facial proportions.
Patients wanting matching dimples on both cheeks, symmetrical facial aesthetics
Unilateral Cheek Dimpleplasty
Single SideA single dimple created on one cheek only. Chosen by patients who prefer an asymmetric, characterful appearance — or to match a naturally occurring dimple on the opposite side that is less defined.
Single dimple preference, matching a natural dimple, unique aesthetic
Chin Dimple Creation
Cleft ChinA vertical indentation created at the pogonion (chin midpoint) — replicating a natural cleft chin or mentalis dimple. The mentalis muscle and overlying soft tissue are sutured to create a permanent midline groove in the chin. A distinct look associated with strong, defined facial structure.
Desire for a cleft chin, stronger chin definition, masculine or bold facial aesthetic
Non-Surgical Dimple (Filler)
Temporary · No SurgeryHyaluronic acid filler injected strategically around a pinched cheek point to create the illusion of a dimple shadow. Completely reversible with hyaluronidase. Results last 6–9 months. Suitable for patients who want to trial the look before committing to surgery.
Trial before surgery, temporary dimple effect, needle-only preference
Your Dimple Creation Journey
Consultation & Marking
Facial analysis to determine the ideal dimple position based on your cheek structure, facial width and golden ratio proportions. Dimple placement is marked on the skin with the patient smiling to confirm positioning looks natural. Bilateral symmetry checked from multiple angles.
Local Anaesthesia
Topical anaesthetic cream applied to the buccal mucosa (inner cheek lining) followed by a local anaesthetic injection. The procedure is performed entirely under local anaesthesia — no general anaesthesia, no sedation. The area is completely numb within minutes.
Intraoral Incision & Suturing
A tiny incision (2–3 mm) is made inside the mouth on the inner cheek — no external incision, no visible scar. A small circular core of the buccinator muscle and subcutaneous tissue is removed. An absorbable suture (Vicryl) is passed through the dermis, around the muscle, and tied — tethering the skin inward to create the dimple indentation.
Immediate Result & Aftercare
The dimple is visible immediately — even at rest — due to post-procedure swelling. As swelling resolves over 2–4 weeks, the dimple transitions to appearing only when smiling, which is the natural look. Chlorhexidine mouthwash used for 1 week. Soft diet for 3–5 days. The entire procedure takes 20–30 minutes.
Why Some People Have Natural Dimples
Natural cheek dimples are caused by a cleft or bifurcation in the zygomaticus major muscle — the muscle responsible for lifting the corners of the mouth during smiling. Where the muscle splits, the overlying dermis is tethered downward, creating the characteristic indentation.
Week by Week Recovery
Dimpleplasty recovery is minimal. Most patients return to work the same day. The main adjustment is understanding the early at-rest dimple appearance — which is temporary and transitions to a natural smile-only dimple within 2–4 weeks.
Mild swelling inside the cheek. Dimple visible even at rest — this is normal and temporary due to oedema tethering the skin. Mild soreness managed with paracetamol. Begin chlorhexidine mouthwash.
Swelling gradually reduces. Dimple still present at rest but beginning to soften. Soft diet continues. No vigorous mouth movement or strenuous exercise.
Most swelling resolved. Dimple now primarily visible when smiling — the natural dimple behaviour. Internal sutures beginning to dissolve. Presentable for social settings.
Dimple fully transitioned to smile-only appearance. Fibrosis (scar tissue) forming between the dermis and underlying muscle — this is what makes the dimple permanent over time.
Final result established. The fibrotic adhesion between skin and buccinator muscle is mature. Dimple appears naturally with smiling and facial movement. Permanent result.
Good Candidates & Things to Consider
Ideal Candidates
- Anyone desiring cheek or chin dimples — no medical indication required
- Adults of any age with adequate cheek soft tissue volume
- Patients who want a permanent result without general anaesthesia
- Those wanting to enhance a naturally faint dimple on one side
- In good general health with no active dental or oral infections
- Realistic expectation: dimple appears with smiling, not a static indent
Things to Consider
- Active oral infection, dental abscess or mouth ulcers at time of procedure
- Expecting the dimple to appear at rest permanently — it transitions to smile-only
- Patients on blood thinners who cannot be safely paused
- Very thin cheeks with minimal subcutaneous tissue — less natural result
- Those seeking a reversible option — non-surgical filler dimple is better
Not sure about surgery? Try the non-surgical filler dimple first — completely reversible, zero downtime, same-day results.
